Designed to complement clinical care and cardiac rehabilitation, this 12-week program empowers patients to self-manage their condition and live well with heart disease, with:

  • 24/7 access to trusted online information and resources on topics including healthy eating, managing medicines and emotional wellbeing, all tailored to the rural and remote experience
  • on-demand videos featuring clinical experts and others living with coronary heart disease, as they share their personal stories of navigating their condition in rural and remote areas
  • peer support, via access to a thriving Facebook community of over 6,000 members where people can connect with others from around Australia who understand what they’re going through.

Already, hundreds of people living in rural and remote areas have benefited from participating in the program. Early evaluation shows that since joining the program, participants are improving their knowledge and understanding of their condition, making changes to improve their diet, and are experiencing fewer unhealthy days.
