Caveat (from Medicare Australia)
"As these forms are for practice software purposes they look different to the standard Indigenous Health Incentive Medicare Australia forms. The recipient and users of these forms are not permitted to alter these forms without approval from the Incentives Program Section, Medicare Australia. Unapproved versions of these forms or versions where the fields and/or wording has been changed will not be processed. The software vendor must not publicise or use Medicare Australia's branding to promote themselves as this would possibly give them a unfair market advance that the government could perceive as supporting as it has our branding. The full suite of interactive forms relating to the Indigenous Health Incentive are available on PIP forms and guidelines page of the Medicare Australia website."
