How to apply for a Clinical Assessment Interview

1. The application Form
Download and complete the CAI application form:

CAI Application Form 

2. A comprehensive CV (in the MBA approved format at Appendix 2 of the M1SP form)

3. A copy of your passport (the original will be required to be presented at face to face interview).

4. A description of the position for which registration is being sought (using one of the following MBA forms):

-           General Practice

-           Private Hospital Non-Specialist

-           Supplementary Information for Queensland Health Position

5. A signed delcaration
